GLP -1 Agonists

GLP-1 is a hormone that your small intestine makes. It has several roles. GLP-1 affects areas of your brain that processes hunger and feeling full. GLP-1 agonists are a class of medications that help manage blood sugar levels in people with Type 2 diabetes. GLP-1 agonists also have been found to help with weight management and to treat obesity.  GLP-1 agonists are most often injectable medications, meaning you inject a liquid medication in the fatty tissue under your skin with a needle and syringe. GLP-1 agonists are medications like semaglutide and tirzepatide (Ozempic, Wegovy, Mounjaro, Zepbound).

You are not a good candidate for a GLP-1 agonist if you have any of the following: diabetic retinopathy, low blood sugar, decreased kidney function, pancreatitis, medullary thyroid cancer (or a family history of medullary thyroid carcinoma), or multiple endocrine neoplasia type 2

These medications work in several ways but, simply put, they slow digestion and increase how full you feel after eating.

 

Phentermine (Adipex)

Phentermine or Adipex, is a well-established medication that has been successfully used to treat obesity for over 60 years. It was approved by the Federal Drug Administration (FDA) in 1959. Since that time, it has helped millions achieve their health and fitness goals.

Phentermine is an appetite suppressant, meaning it decreases the sensation of hunger, cravings, and binge eating. However, it is also a Central Nervous System stimulant, meaning it helps to increase energy level.

You are not a good candidate for phentermine if you have uncontrolled hypertension, glaucoma, hyperthyroidism, a history of heart attack and/or stroke, irregular heartbeat, are currently pregnant or breastfeeding, or if you take a medication of the MAOI class.

Phentermine is not a simple or quick fix to being overweight. However, it is intended to assist you in your weight loss regimen which includes incorporating healthier nutrition and lifestyle habits.

 

Phendimetrazine (Bontril)

Phendimetrazine, also known as generic Bontril PDM, is a well-established medication that has been successfully used to treat obesity for over 40 years. It was approved by the Federal Drug Administration (FDA) in 1976. Phendimetrazine is an appetite suppressant, meaning it decreases the sensation of hunger, cravings, and binge eating. It is also a Central Nervous System stimulant, meaning it helps to increase energy level. 

How is phendimetrazine different from phentermine? Phendimetrazine and phentermine are very similar medications, however phendimetrazine has a shorter half-life, which means it has a shorter duration than phentermine. However, due to this, it can be taken up to three times daily if needed, whereas phentermine can not. Also, due to the shorter duration of action, it decreases the side effect of insomnia which can occur with phentermine when it is taken too late. 

You are not a good candidate for phendimetrazine if you have uncontrolled hypertension, glaucoma, hyperthyroidism, a history of heart attack and/or stroke, irregular heartbeat, are currently pregnant or breastfeeding, or if you take a medication of the MAOI class.

 

Qsymia

Qsymia, approved by the FDA in 2012, is a once-daily prescription weight loss medication that contains two ingredients to help you lose weight and keep it off. Qsymia’s patented formulation is a unique combination of Phentermine and Topiramate, which works synergistically to help decrease appetite, increase energy level, and increase satiety or the feeling of being full.Topiramate, also known as topamax, has been prescribed for many years for seizure disorders and migraine prophylaxis. It can aid in weight loss by making foods (sugars and sodas) taste less appealing while increasing satiety.

You are not a good candidate for Qsymia if you have uncontrolled hypertension, glaucoma, hyperthyroidism, a history of heart attack and/or stroke, irregular heartbeat, are currently pregnant or breastfeeding, or if you take a medication of the MAOI class.

 

Metformin

Metformin has been FDA approved and used for the treatment of diabetes and polycystic ovarian syndrome (PCOS) for over 25 years.  If an individual has insulin resistance, whether from pre-diabetes, type II diabetes or PCOS, the sugar from the carbohydrates consumed are not properly utilized for cell energy and will be stored as body fat. Metformin assists with the regulation of blood sugar and improvement of insulin utilization. Therefore, it will help leave less sugar and insulin around in the body to aid with weight loss.  This will help to “level the playing field” if insulin resistance is prohibiting weight loss efforts. The effectiveness of metformin is not restricted to those with insulin resistance but may be more effective specifically for those individuals.
